This site puts one facility’s whole record on one page, in words: its ratings and why any is missing, every citation and how serious, every fine, who owns it, and what changed this month. 1,426 carry CMS’s abuse icon; 525 are in or a candidate for the Special Focus Facility program; fines on record total $456,752,787.
